About us
To adhere to the Interoperability and Patient Access Rule the Community Health Choice, Inc has implemented the Azure API for FHIR Application by Microsoft Healthcare which enables rapid exchange of data through Fast Healthcare Interoperability Resources (FHIR) APIs, backed by a managed Platform-as-a Service (PaaS) offering in the cloud.
To accommodate this rule the IT department has created Developer Portal to grant access to our Members, Third Parties and Payors via AZURE’s APIM (API Management) to our Azure SMART On FHIR Application. This will allow members to access their health information via the Azure SMART on FHIR application.

Patient Access
This API set supports 3rd party developers building applications that access data on behalf of a member in accordance with the CMS Interoperability and Patient Access final rule (CMS-9115-F)
Payer to Payer
This API set supports members requesting the transmission of their data to a new payer from their previous payer in accordance with the CMS Interoperability and Patient Access final rule (CMS-9115-F)
Provider Directory
This publicly available standards-based API set supports access to Community Health Choice Health Provider Information in accordance with the Interoperability and Patient Access final rule (CMS-9115-F)
